Q: WHAT FORMS OF PAYMENT DO YOU ACCEPT ?
A: You may pay credit card at the time of booking. For your convenience, you will be asked to leave a confidential credit card on file for future bookings.
Q: DO YOU TAKE INSURANCE ?
A: Not at this time. We are considered “out-of-network” and fee for service type of practice. This means that you pay the full amount at the time of appointment booking. For more information, please contact us.
Q: DO YOU COMPLETE PRIOR AUTHORIZATION REQUESTS FROM THE PHARMACY OR PATIENTS ?
A: Unfortunately, we do not complete prior authorization requests/forms at this time, given the fact that the patient is the primary point of contact for their own insurance company as opposed to our practice.
Q: WHAT HAPPENS IF I CANCEL OR MISS ANY SCHEDULED APPOINTMENTS ?
Full 100% refund: If canceled 15 days or more in advance from date/time of appointment.
Partial 50% refund: If canceled, anywhere from 3 to 14 days in advance from date/time of appointment.
No refund: If canceled, less than 72 hours in advance from date/time of appointment.
