Q: WHAT FORMS OF PAYMENT DO YOU ACCEPT ?

A: You may pay credit card at the time of booking. For your convenience, you will be asked to leave a confidential credit card on file for future bookings.

Q: DO YOU TAKE INSURANCE ?

A: Not at this time. We are considered “out-of-network” and fee for service type of practice. This means that you pay the full amount at the time of appointment booking. For more information, please contact us.

Q: DO YOU COMPLETE PRIOR AUTHORIZATION REQUESTS FROM THE PHARMACY OR PATIENTS ?

A: Unfortunately, we do not complete prior authorization requests/forms at this time, given the fact that the patient is the primary point of contact for their own insurance company as opposed to our practice.

Q: WHAT HAPPENS IF I CANCEL OR MISS AN APPOINTMENT ?

A: If you contact us within 48 business hours of your appointment you may reschedule with no additional cost, and we will offer the appointment time to someone else. If you cancel the appointment within 48 hours of your appointment or if you miss your appointment, we are unable to offer the appointment to someone else so you will be charged the full amount of the appointment.
